We ask that patients provide adequate notice when cancelling or rescheduling an appointment. 4. CASH-PAY & PAYMENT TERMS

Core Physical Therapy is a cash-based practice. Payment is due at the time of service unless other arrangements have been made in advance. We accept cash, credit and debit cards, HSA, and FSA payments.

While we provide a superbill after each session that you may submit to your insurance provider for potential reimbursement, we do not bill insurance directly and cannot guarantee reimbursement. It is your responsibility to understand your insurance plan’s out-of-network benefits.
